Winners may be required to present state photo ID, social security number and sign a liability release. Failure may result in forfeiture of the prize.

Prizes must be claimed within 14 days (two weeks) of being notified that the prize is available for pick-up. Any prizes not claimed within 14 days shall be considered forfeited and will be redistributed.

Winner responsible for all taxes.

Only the WINNER with proper Picture ID can pick up the prize. Friends or Relatives are unable to pick up a prize for someone.
